Rizwan, a 26-year-old resident from Karachi, has been feeling pain and swelling around his anal area. His wound around the anal opening has been painfully red, sore, and itchy many times for the past six years. In search of solace, he went to see a general surgeon, who confirmed his fears about having a perianal fistula. The doctor explained the seriousness of the situation, emphasizing the importance of diagnostic tests to confirm the diagnosis. Following a thorough examination, the doctor advised him to have a fistulectomy, a surgical procedure to treat the condition.

He is living with a joint family, which consists of eight members. He is doing a private job and earns just PKR 20,000 per month.
