Muhammad Matloob, a 54-year-old resident of Karachi, has been suffering from pain, swelling, and discharge from the lower back and buttock area. He went to the local hospital, where the doctor prescribed pain management and antibiotics. But his condition did not improve. Later on, he is diagnosed with pilonidal sinus. It is a condition in which a small tunnel or tract forms under the skin at the top of the buttocks, near the cleft of the buttocks. This tunnel or tract contains hair follicles, debris, and sometimes bacteria, which can cause infection and inflammation. The doctor advised him to undergo pilonidal sinus surgery. It is a surgical procedure that involves the removal of a pilonidal cyst or sinus. 

As the sole breadwinner of a family of six, he earns a meager income of only PKR 28,000 per month. This places a significant financial burden on him, as he must support his family’s basic needs on a limited budget. The cost of the surgery is beyond his affordability. He said it in despair.
